Title: Knowledge and attitude of undergraduate medical students in kenya towards solid organ donation and transplantation

Abstract:

Objective: To describe the knowledge and attitudes of Kenyan undergraduate medical students towards solid organ donation and transplantation, and their level of competence in handling end-of-life issues. Methods: A sample of undergraduate medical students from all the medical schools in Kenya offering a Bachelor of Medicine and Bachelor of Surgery (MBChB) were prospectively surveyed using a self-administered web-based questionnaire, between July and September 2018.Results: There were 303 participants; 167 (55.1%) were female and 136 (44.9%) were male. Only 8.9% of the students had read the laws governing organ donation and transplantation in Kenya. An even lower percentage (3.3%) felt that they had learnt enough about solid organ donation from their MBChB curriculum. 52.8% of the respondents would subscribe as solid organ donors; which reduced to 46.5% when it came to consenting the donation of their relative’s organs. Less than half of the students felt they would be comfortable introducing the topic (39.6%) or confident answering questions (22.8%) related to organ donation and transplantation. Only 9.9% of the students had ever spoken to a patient about organ donation. There was no significant association between level of study (pre-clinical versus clinical) and comfort introducing the topic of organ donation (p=.206) or experience talking to a patient about organ donation (p=.102).Conclusion: Undergraduate medical students have significant knowledge gaps regarding organ donation and transplantation, and feel ill-prepared to approach a potential donor or transplant recipient. Introduction of dedicated organ donation education programs targeting medical students can be used to address these deficits.

Title: What's the choice between radical nephrectomy and simple nephrectomy or partial nephrectomy in management of localized renal cell carcinoma.

Abstract:

Purpose: The purpose of this paper is to review available evidence to compare oncologic outcomes between partial nephrectomy and radical nephrectomy for localized renal cell carcinoma, and to compare clinical efficacy of radical nephrectomy with simple nephrectomy in treating patients with localized renal cell carcinoma. Methods: Through searched several public databases including pubmed , MEDLINE , Springer , Elsevier Science Direct , Cochrane Library , and Google Scholar, The key words "Radical Nephrectomy" , "Simple Nephrectomy" , "Nephron Sparing Surgery" , " Partial Nephrectomy" , "Renal Cell Carcinoma" ," Renal tumor" , and " Study " Or " Trail" were used to retrieve the potentially relevant literature. A Publication met the predetermined search strategy. Full publications were obtained and reviewed to determined how this research is being used in education, research, and practice.Finding:  A Comparison studies have been conducted on a variety of research instruments , between RN and PN with oncologic outcomes (local ipsilateral recurrence , distant metastasis , and cancer specific mortality [CSM] ) and clinical outcomes(other cause of mortality[OCM], > 10% decrease in estimated glomerular filtration rate[CKD10%] , and decrease in estimated glomerular to < 45 ml/min/1.73 m² [CDK,45] ). RN was not associated with significant difference in the development of distant metastasis and CSM. Although RN was associated with an increased risk of CDK10% , and CDK,45, it was not associated with OCM or ACM.Conclusion: Since the publication of Vincent Vermooten paper from five decades show how correct was his suggestion to use partial Nephrectomy in a patient with a Healthy contralateral kidney. To date, for T1a renal cell carcinoma and is a suitable option for cT1b tumors . Simple nephrectomy seems to be as effective as radical nephrectomy in tumors up to 4cm.

Title: Polymorphism of rs2228145 and rs599839 as a Predictor in Development of Abdominal Aortic Aneurysm in Germany Population

Abstract:

Background AAA is a degenerative vascular disease. Predominant risk factors are smoking, male, family history of AAA, and CAD. Reports indicate that two proteins are significantly up- regulated in the aortic wall of AAA, sortilin-1 responsibles for circulating lipoprotein and interleukin 6 (IL-6) a pro-inflammatory interleukin. An SNP of sort1 rs599839 A>G in vicinity of gene encoding sortilin 1 and il6r rs2228145 A>C might play a significant role in the development of AAA. This study aims to identify the predictive power of those SNPs for the personalized risk assessment for developing AAA in the Germany population. Furthermore, with the determination of the single nucleotide polymorphism of selected gene, will detect the probability of AAA in the futureSingle-centered case-control study with AAA patients diagnosed >40 mm in diameter and healthy individuals as a control were recruited after approval of ethical committee. The DNA was isolated from blood in PAXgene DNA (PreAnalytiX) with the PAXgene DNA Kit. PCR products were purified (PeqGOLD Kit, VWR) and sequenced according to Sanger (GATC Eurofins). The target sequences were analysed by SNAPGene (GSL Biotech LLC), Clustal Omega (EMBL-EBI). Statistical analysis was performed by SPSS 25 (IBM) and HaploView (Broad Institute) was used for the Linkage of Disequilibrium (LD) detectionIn total 92 individuals were evaluated (56 patients). The 80.4% of patients were male, 57.1% were smoker and 16% had relatives with AAA. Mean of age, Body Mass Index (BMI), and diameter of aneurysmal of patients group were 72±7.7 years, 27.1±4.3, and 56.7±11.5 mm, respectively. Sort1 SNP was identified as wild homozygote in 84.8% (78/92), heterozygote in 15.2% (14/92), and rare homozygote in 0% (p value .04). The minor allele frequency (MAF) for patient was .09, and for control was .06. For il6r SNP was in the same amount with sort1 SNP, nevertheless the MAF for patient was .04, and for control was .13. The LD was D’=.08 (r2=.006).
